How school teams use them
Start the checklist when consent is signed, so the team can see which measures are planned and who is giving them. Update dates and results as testing happens, then bring the completed checklist to the eligibility meeting to confirm every requirement has supporting data.
Do these replace state or district forms?
No. They are working checklists to organize evaluation data. States may add criteria or use different terms, so always follow your state and district procedures and required forms.
